Sickly Sister — Append 02 (A Sex-obsessed Brother in the Country, and his Traditional, Sickly Sister Append.02) — Honest Review

Append 02 is a small bolt-on for A Sex-obsessed Brother in the Country, and his Traditional, Sickly Sister, adding three new CGs and eight new scenes built around one very specific joke: the protagonist’s ever-expanding repertoire of “home remedies” for a sister whose body keeps finding new ways to be fragile. It is content for people who already own the base game, already like Asumi, and want more of exactly what they already have. If you’re looking for a way into the series, this isn’t it — the base game is required, and this patch assumes you’re already invested.

What works

Sickly Sister — highlight scene

The premise for this append is well-chosen, and that’s most of the battle for a DLC this size. The hook the description leans on is that Asumi’s long hospitalization left her with more than asthma — she gets overheated in the bath, her circulation is bad, she has small dizzy spells, and generally her body is a list of everyday complaints rather than one dramatic illness. That’s a smart engine for episodic scenes, because it gives the writer eight different excuses for physical intimacy that all come from the same place: the brother trying, in his self-taught and very obviously motivated way, to make her feel better. The comedy is baked in. So is the tenderness. The description even winks at it — “as to how well they’ll work” — which tells you the work knows exactly what kind of thin pretext it’s operating on, and is having fun with it rather than pretending otherwise.

Mechanically, the append targets the first half of the main story, Tiers 1–2, and is billed as adding high-efficiency events there. That’s a meaningful choice rather than a lazy one. Early-game stretches in RPG-structured works like this are usually the least eventful part of the run — you’re grinding stats or progress toward the interesting material — so dropping new, efficient events into that window is aimed squarely at the game’s real weak spot instead of piling more content onto an already-crowded endgame. Whether the efficiency figures are actually generous is something the description doesn’t quantify, but the intent is the right one.

The production side stays consistent with the base work, which matters more than usual for an append — nothing kills an add-on faster than art or voice that doesn’t match. K-ko is back on artwork, Azumahito on scenario, Chata directing, and the voice recording is handled by a professional studio (Studio Unshakable). The result is that the new material sits inside the existing game rather than beside it. Full Japanese voice work carries the scenes, and given how much of Asumi’s appeal is in delivery — the healing, sweet-tempered, black-haired, slender, quietly-attached archetype the tag list is openly advertising — voice is where most of the value lives. The three new CGs are pleasant but they’re not really the point; the eight scenes and their audio are.

And the whole thing is unabashedly a pure-love, lovey-dovey, slice-of-life work. There’s no NTR shadow, no dark turn, no drama being set up to hurt you later. If the appeal of the base game was that it’s a warm, low-stakes domestic loop with a girl who is genuinely fond of you, Append 02 is more of that loop, and it doesn’t betray the tone.

What doesn’t

Sickly Sister — drawback example scene

It’s small, and it’s honest about being small: three CGs, eight scenes. That is a modest amount of content, and if the base game already gave you dozens of scenes, this is a thin slice by comparison. There’s also a structural annoyance built into the design decision I praised above — because the new events sit in Tiers 1–2, the first half of the story, players who have already completed their run may need to go back through early-game content to reach them. The description doesn’t say whether there’s a shortcut, a gallery unlock, or any way to view the new scenes without replaying up to that point, and for a DLC that’s a real question worth asking before you buy rather than after.

The other honest note is thematic: the “home remedy” framing is a pretext, and eight scenes’ worth of pretext is a lot of weight for one gag to carry. Bath-overheating, cold hands, a dizzy spell — these are all variations on the same setup, and if the base game already ran through a similar sequence of “she’s unwell, you help her, one thing leads to another,” Append 02 is going to feel less like new territory and more like the same room with the furniture moved. That may be exactly what you want. But it isn’t expansion; it’s refill.

Who should buy this

Sickly Sister — target audience scene

Buy this if you already own and enjoyed the base game and want more Asumi — that’s genuinely the entire audience, and the store page says as much. English text is supported (alongside Japanese, Simplified and Traditional Chinese, and Korean), with Japanese voice, so non-Japanese readers can play it without any patch hunting or machine translation. If you don’t own the base game, buy that first and come back here later; this won’t work on its own and won’t make sense on its own.

Verdict

Sickly Sister — final verdict visual

7 / 10 — a well-targeted, well-produced append that adds exactly what it promises to the part of the game that needed it most, held back only by its small size and the unanswered question of how much replaying it takes to actually see it.
